Difficulty improvements

Update in the difficulty department!

• Better difficulty curve
• Less speed increase overall, giving more actual changes of higher scores
• Better "difficulty floors" where new item types are introduced with a small decrease in the number of items spawned over time
• Improvements to not lose all your hearts immediately after a small mistake: losing a heart now clears all items which were placed on the board, giving you a real chance to recover
